Storyboards for Maths

Hi,
I am looking for a simple template for a storyboard that I can use with my junior primary class. I want to use it in maths. The template I have seen somewhere (but can't think where), is an A4 divided equally into 4 areas with a square in the center.
Each 1/4 has a different title and role eg: picture, story, materials, symbols. For my younger students I would simply write a number in the central box eg 5. They then have to explore that number through pictures (draw 5 items), symbols (write the number 5), or with the use of materials (counters, popticks etc). For my older students I would write a sum in the centre and they have to explore the different ways that sum can be represented.
Can anyone suggest a site that may have such a downloadable template?
Thank you.

What others are saying...
This site has some good ladybug maths ideas. I laminated the large ladybug and had the children see how many ways they can put her 10 dots (counters) back. They write 10 up above her head and the two numbers to make 10 under her wings-Use textas. Then they wipe the mat clean and try another way. The prep students loved this activity.

Hi,
Thanks to everyone for your ideas and also to Penne for your offer to email your template to Donna for me to use. That would be great. I like the idea of using ten frames and MAB tens and units blocks on them too. I've since learnt that they are also called 'Thinkboards'. Thanks also Donna for 'throwing' something together! Yes, as usual, I'm sure it will be 'just right'!
Cheers!
Hi Julia.
I use these in my classroom. I will see if I have one saved on my computer at school and email it to Donna and then she can pass it onto you.
Really easy to do one in a word document. I have also had kids use it interactively in the kid pix program in my grade one class. I'm sure Donna will come up with something amazing!
We use a similar tool but it is a circle inside a larger circle with the outer area divided up. Have seen it with different headings depending on the age group it is used with and included things like place value blocks, tens frames. The kids really enjoy filling them in and often they work in pairs and the conversations about what they are doing are really helpful for me in assessing where their thinking is at.
Hi Julia
That's a great idea and doesn't sound too difficult - we will try and 'throw' one together for you today or tomorrow!
Cheers
Donna
